What is long-term care? Provide an example.
Long-term care is when someone provides a variety of services that do includes medical and non-medical care to peoples that who have a bad chronic illness and a disability. The long-term care helps to meet with your health and personal needs. The most of your long-term care is to provide people with some support services and this is such as activities on a daily living like dressing, bathing, and feeding, and using the restroom. The long-term care can be given to at home, in your surrounding area (like your community), can assisted in-living, and nursing homes. Long term care is always given to people at any age range it really all depends on your condition and situation. For instance, Say I had a patient and both of her legs were cut off for diabetics that patient would need to be feed, bathing, dressing, and driven to places that she need to go. If her condition starts getting worse the patient would need a private nurse to come on duty to come in the patient home to take care of them.

What is the continuum of care services in the United States?
The continuum of care services is a big concept that is involving the integrated system of care that will guide and tracks your patient over time through on health services that spanning all around levels of intensity care. Continuum care is a providing health care service that vital the component. The service provides you with anything you need. The care is there to manage you effectively through your care, when it comes to prescription and making your doctor appointments and hospital stays, the critical care (ICU) they are there to manage your care. Every staff and your doctor who involved with your care will keep an update on everything so you want be getting the same service over again.
